Location: Flagstaff, Arizona
Schedule: Monday–Friday, 8-hour daytime shift (flexible options: 7am–3pm, 8am–4pm, or 8am–5pm)
Job Summary
We are seeking a skilled Wound Care Nurse (RN or LPN) to provide expert wound assessment, treatment, and ongoing management for residents in a post-acute/long-term care environment. The clinician will support wound healing, prevent skin breakdown, and serve as a resource for staff education and best practices in wound and ostomy care.
Key ResponsibilitiesPerform wound assessments, reassessments, and treatment in accordance with established policies and procedures
Develop and oversee skin integrity programs to prevent resident skin breakdown
Participate in creating and implementing policies related to wound care, wound prevention, ostomy care, and incontinence management
Update care plans based on resident response and evaluate treatment effectiveness
Assess and reassess pain; implement appropriate pain management strategies
Educate residents, families, and staff on wound care and pain management
Provide in-service training on wound prevention, wound management, ostomy care, and incontinence
Communicate clearly and effectively with nursing staff and physicians
Collaborate with interdisciplinary team members as needed
Ensure documentation meets current standards and regulatory requirements
Maintain up-to-date knowledge of wound care best practices through continuing education
Active Arizona RN or LPN license
Certification as a Wound, Ostomy, and Continence Nurse (CWOCN) or Certified Wound Care Nurse (CWCN)
Ability to obtain and maintain a Level 1 Fingerprint Clearance Card (Arizona Department of Public Safety)
Current CPR card obtained through in-person assessment
Ability to pass all required health screenings and background checks
Previous experience in a Skilled Nursing Facility (SNF) or Long-Term Care (LTC) setting
